Supporting Documentation for Registration
Government-issued ID
You must provide 1 of the following documents
Passport
Requirements
- Must include your picture
- Must include your full name
- Must include your date of birth
- Must not be expired
Birth Certificate
Requirements
- Must include your full name
- Must include your date of birth
Other Government ID
Requirements
- Must include your full name
- Must include your date of birth
- Must not be expired
- Must include your photo
Please note
If your government-issued ID doesn’t have a photo, you must include a full-colour passport sized photo taken within the last 6 months, that has been notarized on the back.
CF1 Card or Canadian Forces Certificate of Service
Requirements
- Only required for Canadian Military Status Applicants
- Must include your full name
- Must not be expired

Transcript
Requirements
- Required if applicant is 14-20 years old or applying for OSSD through TVO ILC
- Must include full name, date of birth
